Geriatric Care Management Pathways for Aging supports older adults by providing in home assessments for a range of different needs as well as recommendations, assistance, monitoring and reporting for a loved one. Assessments and all services are provided in the home of the older adult whether they live in a private residence, senior apartments or skilled nursing facility. A comprehensive assessment includes:
- physical abilities, limitations, hazards in the home, and safety
- ability to function physically or cognitively in the home
- cognitive ability, decision-making capacity & memory loss
- mental and emotional health status including Depression, Anxiety, and Dementia
- financial, eligibility for entitlement programs, health & long-term care insurance and legal needs
- community and emotional support network
Our Social Workers provide:
- a written, comprehensive biopsychosocial assessment of the older adult or couple
- professional recommendations and options for loved ones
- assistance in locating appropriate resources and coordinating services
- assistance in managing paperwork, insurance claims and benefits
- customized information and Financial Aid as our staff evaluates eligibility for services & resources
- peace of mind if you are separated from your loved one by geography, as we check-in on your loved one regularly
- someone to act as your eyes & ears to troubleshoot problems before they become a crises and provide ongoing monitoring and reporting to caregivers
- a supportive advocate when making life-altering or medical decisions & when determining whether a loved one is able to continue living independently or consider alternative housing
- professional help with strained family relationships and decisions
Comprehensive In-Home assessments, including a written report and family conference/feedback are $350.
One Hour Telephone Consultations help you develop an action plan and focus your efforts in locating services and resources. These can be scheduled for $75.
Care Management is $100 per hour and can be provided intensively for a short-term issue or regularly on a weekly, bi-weekly, or monthly basis.
The number of hours and service provided are tailored to meet each client or family’s specific needs.
