For Professionals Making a Referral

Counseling/Psychotherapy

For clients in need of weekly counseling for depression, anxiety, or other mental health concern:

• Discuss with the client their willingness to meet weekly with a clinical social worker for an hour
• Collect the following information for the referral: Client name as listed on their Medicare Card, Address, Phone Number, Date of Birth, Social Security Number, Medicare Number, Secondary Insurance Information
• If the above information is in print form, FAX the FACE-SHEET, a copy of the INSURANCE CARDS, and a copy of the PHYSICIAN’S ORDER (if living in a facility) to 314-395-7563
• If faxing is not possible, call 314-395-7560 to give referral information over the phone
• If the situation is complex, and you want to discuss the case before making a full referral, call our office at 314-395-7530 to request a phone consultation from our Intake Social Worker

Geriatric Care Management

For clients and caregivers in need of support with decision-making, connecting to resources, on-going  monitoring, or recommendations on difficult issues:

• Discuss with family member, caregiver or older adult the need for additional support from a care manager
• Call our office or invite the caregiver to contact our office to discuss personalized care management and set-up a FREE initial consultation for their unique situation at 314-395-7560
