Pathways for Aging serves older and disabled adults currently living in the St. Louis, Missouri Metropolitan Area. Our service area includes St. Louis City and County, St. Charles County, and Northern Jefferson County. We also have a Geriatric Care Manager serving Southwest Illinois and hope to provide psychotherapy to the Metro-East area in the near future. We are able to work with family members who live out of state if their relative currently resides in the St. Louis area or needs assistance moving to the metropolitan area.

Pathways for Aging provides in-home or in-facility counseling and therapy specifically intended for older adults and caregivers. Therapists serve as supportive listeners who work with individuals to navigate their new roles and emotions that often come with aging. Our therapists utilize a wide variety of techniques including behavior modification, reminiscence therapy, supportive therapy, anxiety reduction, and problem solving therapy. We also provide services for groups and organizations.

Pathways for Aging offers:

  • Assistance and support for caregivers
  • Counseling and psychotherapy services for older and some disabled adults
  • Support and guidance in making difficult end-of-life decisions
  • Support groups tailored to your organization

Geriatric Care Management

Pathways for Aging supports older adults and caregivers by performing in-home assessments, recommendations, assistance, monitoring and reporting for and older adult. Assessments and all services are provided in the home or senior housing facility of the older adult. A comprehensive assessment includes:

  • Physical abilities, limitations and safety
  • Ability to function in the home
  • Cognitive ability, decisional capacity & memory loss
  • Mental and emotional health status
  • Financial, insurance and legal needs
  • Community and emotional support network

Pathways for Aging provides:

  • Recommendations and options for your loved one
  • Assistance in locating appropriate resources and coordinating services
  • Ongoing monitoring
  • Regular reporting to caregivers
